I am the parent/legal guardian of the athlete/child listed below and I grant Rockford Cheerleading Club (NFP) my permission to use my child’s photograph publicly to promote the Rockford Phoenix Cheerleading Club (NFP). I understand that the images may be used in print publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me or my child by reason of such use.
